Extrachromosomal elements (ECEs) are major components of the human microbiome, expanding the gene repertoire of host bacteria to alter their traits. We developed long-read metagenomics technology coupled with an efficient microbial DNA extraction method from saliva samples and discovered a previously unrecognized family of ECEs. These ECEs reside in 68% of the human oral microbiome globally. We refer to this ECE as âInocleâ, characterized by insertion sequences encoded, oral origin, and circle genomic structure. Inocles are plasmid-like elements of an average genome size of 330kb, having Streptococcus as a host bacterium, and encode a series of genes related to stress tolerance and cell wall modification. Moreover, Inocles showed significant associations with immune-related cells and proteins in peripheral blood and marked reductions in patients with certain cancers. These findings suggest that Inocles potentially boost the adaptive capacity of host bacteria in the oral environment to fit the physiological conditions of humans.
